As of December 26, 2015, we implemented a new digital Forever Waiver. Gone are the days of filling out a paper waiver at registration every weekend. Now, enjoy the "One and Done" Forever Waiver!

Tips for Completing the D14 Airsoft Forever Waiver
- If you're under 18 years of age, the top portion of the waiver must be completed by a guardian who is 18 or older.
- Only one player can be listed on each waiver. If you are a player and a guardian, please complete a second waiver for yourself.
- Please provide a valid phone number and email address. This information will ONLY be used in the event of an emergency or field related matter. We never sell your information.
- Clicking 'Submit' and nothing changes? Scroll up to see if there are any fields highlighted in pink. Common culprits: Pre-filled email addresses can be corrected by removing the space at the end. Don't forget to check the box agreeing to the rules at the beginning of the Player Information.
- Please do not print your waiver. This form is intended to be submitted digitally and kept on file for future visits. Printed waivers are only kept for one day and will not be considered a Forever Waiver.
